Menopause instructions usually concern whether symptoms were recognised as menopausal, whether treatment was offered, and whether the risks and benefits discussed are supported by the record. The evidence base on hormone replacement has shifted over time, so the report should identify the guidance in force at the date of the prescribing decision.
