Menopause instructions usually concern whether symptoms were recognised as menopausal, whether treatment was offered, and whether the risks and benefits discussed are supported by the record. Prescribing entries and review appointments are the main material. The evidence base on hormone replacement has shifted over time, so the report should identify the guidance in force at the date of the prescribing decision.

  • Whether investigation was appropriate to the age and presentation documented.
  • Whether hormone replacement was offered, and whether the discussion of risk and benefit is recorded.
  • Whether prescribing, route and review met the standard expected at the time.
