- Whether the symptoms recorded met the threshold for urgent referral under the pathway in force.
- Whether triple assessment was completed and its results acted upon.
- Whether the operation performed and the margins achieved met the expected standard.
- Whether reconstruction was discussed, offered and carried out appropriately.
- What difference earlier diagnosis would probably have made to the treatment required.
