A stroke instruction should establish the time of onset relied upon, the time of arrival, the time of imaging and the time of any treatment decision. Those four points usually determine whether the outcome could have been different.
- Whether stroke was recognised at first contact against the symptoms recorded.
- Whether imaging was obtained and reported within the period the pathway required.
- Whether thrombolysis or thrombectomy was considered and the decision adequately recorded.
- Whether transient ischaemic attack was investigated and treated to prevent recurrence.
- What difference earlier treatment would probably have made to the deficit sustained.
