A gynaecological instruction may concern delayed diagnosis, operative technique, an intra-operative complication, consent or continuing symptoms after treatment. Operation notes, imaging, histology and outpatient correspondence usually need to be read together rather than in isolation.
- Whether investigation of bleeding, pain or a pelvic mass was appropriate to the presentation recorded.
- Whether the operative approach and technique met the standard expected at the date of surgery.
- Whether a recognised complication was avoidable, and whether it was identified and managed promptly.
- Whether the consent discussion recorded covered the material risks of the procedure performed.
- Whether continuing symptoms are attributable to the treatment given or to the underlying condition.
