Emergency care is judged on incomplete information, and a reliable opinion reflects that. The department record, triage category, observation chart, imaging requests and discharge advice show what was known at each point, which is not the same as what became clear later.
- Whether triage category and initial assessment matched the presentation documented on arrival.
- Whether the history, examination and investigations undertaken were adequate for the presenting complaint.
- Whether imaging, blood tests or specialty referral were indicated by the findings recorded.
- Whether the decision to admit, discharge or refer was supported by the clinical picture and safety-netting given.
- Whether a missed injury or diagnosis was reasonably identifiable on the material then available.

Emergency department attendances are usually well documented, so a records review report can often answer the question without examination. Where the criticism and its consequence must be distinguished, a breach and causation report is the appropriate route.
